چکیده

UNLABELLED BACKGROUND We aimed to minimalize operative complications by spraying of methylene blue stain on thyroid glands and the perithyroidal area. MATERIAL AND METHODS The intra-operative methylene blue spraying technique was used prospectively on a total of 56 patients who had undergone primary (not recurrent) thyroid surgery for a variety of thyroid diseases. Bilateral total thyroidectomy was performed in all cases. After superior but before inferior pole ligation, 0.5ml of methylene blue was sprayed over the thyroid lobe and perilober area. Tissues, especially parathyroides, the recurrent laryngeal nerve, and the inferior thyroid artery, were identified and evaluated. RESULTS Recurrent laryngeal nerve and arteries were not stained and thus they remained white in all cases while all other tissues were stained blue. Within three minutes parathyroid glands washed out the blue stain and the original yellow color was regained. Thyroid tissue wash-out time was not less than 15 minutes; perithyroideal muscles, tendinous and lipoid structures took no less than 25 minutes. CONCLUSION The safety of intravascular methylene blue guidance on thyroid surgery is known. This research demonstrates the effectiveness of the spraying technique, a new technique which ensures not only identification of parathyroid glands within three minutes, but also identification of recurrent laryngeal nerves and inferior thyroid arteries.

Technical refinement of ultrasound-guided transoral resection of parapharyngeal/retropharyngeal thyroid carcinoma metastases.

BACKGROUND The transoral approach to the parapharyngeal and retropharyngeal space (PPS/RPS) for the management of well-differentiated thyroid carcinoma (WDTC) has been previously described in other articles. However, limited exposure with this approach can be a challenge. METHODS This is a retrospective review of 6 patients who underwent ultrasound-guided transoral excision of PPS/RPS WDTC me...

Intraoperative use of fibrin glue dyed with methylene blue in surgery for branchial cleft anomalies.

OBJECTIVES/HYPOTHESIS We present a new method of optimizing the results of surgery for branchial cleft anomalies based on the intraoperative injection of fibrin glue combined with methylene blue dye. STUDY DESIGN Retrospective single-center cohort study. METHODS The method was applied in 17 patients suffering from branchial anomalies. Six (35.29%) had a preauricular lesion; three (17.65%) h...

Frozen section analysis and sentinel lymph node biopsy in well differentiated thyroid cancer

BACKGROUND The aim of this study is to prospectively review the role of sentinel lymph node (SLN) biopsy in the management of well differentiated thyroid carcinoma (WDTC), and to determine the efficacy of intraoperative frozen section analysis at detecting SLN metastasis and central compartment involvement. METHODS The SLN biopsy protocol using 1% methylene blue was performed in 300 patients ...

Prolonged elevation of serum tryptase resulting from intraoperative anaphylaxis to methylene blue

Background Intraoperative anaphylaxis during sentinel lymph node biopsy is a well known phenomenon occurring with administration of patent blue and isosulfan blue dyes, with an incidence of 2.2% and 1.1%, respectively (1,2). Methylene blue has been reported as being a safer alternative with only a few case reports demonstrating anaphylaxis and large studies demonstrating no incidence of anaphyl...

VATS intraoperative tattooing to facilitate solitary pulmonary nodule resection

INTRODUCTION Video-assisted thoracic surgery (VATS) has become routine and widely accepted for the removal of solitary pulmonary nodules of unknown etiology. Thoracosopic techniques continue to evolve with better instruments, robotic applications, and increased patient acceptance and awareness. Several techniques have been described to localize peripheral pulmonary nodules, including pre-operat...
